Eligibility
If you’re requesting academic accommodations, you’ll need disability documentation prepared by a qualified professional and shared with our Accessibility Services Coordinator. The notes below show what to include and how we handle your information.
What to Submit
Provide current documentation that clearly identifies your disability and how it affects learning and campus life.
- Credentials of the evaluator
- Clear diagnostic statement
- Tests/methods used & specific results
- Current functional limitations
- Expected progression or stability
- Relevant educational/developmental/medical history
- Your specific limitations in courses/programs/services
Who Can Provide It
Documentation should come from an appropriate professional—such as a medical doctor, psychologist, or other qualified diagnostician—and be submitted to the Accessibility Services Coordinator.
If You Don’t Have Recent Documentation
North Carolina Vocational Rehabilitation may be able to help if you don’t have documentation or yours is more than five years old.
Confidentiality
Your records are kept confidential in line with the Family Educational Rights and Privacy Act (FERPA). College admissions procedures apply equally to students with disabilities.
