EACH FORM BELOW MUST BE COMPLETED BY THE APPLICANT, SPONSORING AGENCY, OR PHYSICIAN.  EACH FORM MUST BE PRINTED.  AFTER COMPLETION OF ALL DOCUMENTATION BY THE APPROPRIATE AUTHORITY, THE FORMS MAY BE SUBMITTED IN PERSON AT THE LAW ENFORCEMENT TRAINING CENTER AT MCDOWELL TECHNICAL COMMUNITY COLLEGE OR BY CLICKING ON THE "SUBMIT APPLICATION" BUTTON BELOW.  EACH FORM WILL NEED TO BE SCANNED AND SAVED ON YOUR DEVICE IN ORDER TO SUBMIT ELECTRONICALLY.  ADDITIONALLY, PLEASE HAVE A COPY OF YOUR VALID DRIVER'S LICENSE, BIRTH CERTIFICATE, HIGH SCHOOL DIPLOMA, AND CRIMINAL RECORDS CHECKS FROM EACH COUNTY YOU HAVE LIVED SINCE AGE 18.   IF YOU ARE A VETERAN, A COPY OF YOUR DD214 WILL ALSO BE REQUIRED.
